Before your appointment, we may send you a short questionnaire—either by post, text, or email. This helps us understand how you’re feeling and whether you need a face-to-face or phone appointment. In some cases, no further action may be needed.
Please complete and return the questionnaire as soon as possible to help us tailor your care.

Once we’ve reviewed your questionnaire, you’ll receive a link via SMS or email to book your own appointment at a time that suits you. This makes it easier and more convenient for you to manage your care.

🩺 What Is an Annual Review?
An Annual Review is a once-a-year appointment designed to check in on your overall health, especially if you live with a long-term condition such as:
- Diabetes
- Asthma or COPD
- Heart disease
- High blood pressure
- Mental health conditions
It’s a chance to:
- Review your current medications
- Monitor your condition(s)
- Discuss any new symptoms or concerns relating to your long term condition(s)
- Set health goals and support self-management
- Explore lifestyle advice and support options
👩‍⚕️ What Happens During the Appointment?
Your review may be split into two parts:
- Initial Checks
A healthcare assistant or nurse may carry out:- Blood pressure and weight checks
- Blood tests (if needed)
- Lifestyle questions (e.g., smoking, alcohol, exercise)
- Review Consultation
Depending on your needs, you may see a:- Practice Nurse or GP to discuss your condition and care plan
- Clinical Pharmacist to review and optimise your medications, especially if you take multiple prescriptions
Appointments are tailored to your needs and may last up to one hour if you have multiple conditions.

We encourage all patients to use the NHS App to help manage their health more easily. With the app, you can:
- View your test results and upcoming appointments
- Order repeat prescriptions
- Access your medical records
- Receive messages from the practice
